The geography that drives every decision
Phuket is a large island and the airport is at the top of it. Patong, Karon, Kata and Rawai are down the western and southern coasts, Phuket Town is inland to the southeast, and Mai Khao and Nai Yang are the beaches actually near the airport.
That layout explains everything about arrival day. Transfers here are not the ten-minute hop that "island airport" suggests, and where you have booked your hotel determines the length of the drive far more than which vehicle you choose.
The Smart Bus
The Phuket Smart Bus runs a scheduled route between the airport and Rawai via Patong, Karon and Kata, at a flat 100 baht regardless of how far you travel, departing hourly. The operator lists airport-to-Rawai services from 06:30 to 17:30 and northbound services from 06:00 to 17:00.
Those hours are the catch. A flight landing in the evening, which describes a great many arrivals into Phuket, falls outside the published window entirely. Check your landing time against the timetable before assuming the bus is an option.
Limousine counters and taxis
Airports of Thailand publishes limousine counters in the Domestic terminal on Floor 1, in the public area close to both arrival halls, offering sedans, SUVs and minibuses. What it does not publish is a price list: the official page directs travellers to a separate booking site or the airport call centre for rates.
That absence is worth knowing in advance. Unlike Bangkok, where the airport publishes a metered fare structure plus a fixed surcharge, Phuket's arrival hall is a place where you agree a price at a counter. Do it at a counter, indoors, before you go outside with your bags.

If you are arriving in the evening
Evening arrivals are the norm on long-haul routings into Phuket, and they are the case where pre-booking earns its cost. A driver holding a sign after a ten-hour flight, at a counter-free hour, on an island where you do not yet know the geography, is worth more than the difference in price.
Ask your accommodation what they charge and compare it against the airport counters. Many properties arrange this as routine and the rate is often reasonable.
Before you leave arrivals
Confirm the price and the destination before the bags go in the boot, and make sure the driver has the hotel's name and area rather than a screenshot in a language he cannot read. Phuket has multiple properties with similar names spread across very different parts of a big island, and that is a mistake best avoided at the start rather than corrected halfway down the coast.
